To convert from other oral oxycodone formulations to OxyContin® in adults
- Determine the patient's total daily oral oxycodone dose
- Administer one half of the patient's total daily oral oxycodone dose as OxyContin every 12 hours
- Discontinue use of the patient's around-the-clock IR oxycodone when initiating OxyContin therapy
- Monitor patients closely for respiratory depression, especially within the first 24–72 hours of initiating therapy and following dosage increases with OxyContin and adjust the dosage accordingly

Monitor elderly, cachectic, or debilitated patients, and patients with chronic obstructive pulmonary disease or cor pulmonale closely, particularly when initiating and titrating OxyContin and when OxyContin is given concomitantly with other drugs that depress respiration. Alternatively, consider the use of non-opioid analgesics in these patients.
